Responsibility
- Perform comprehensive biopsychosocial assessments to identify client strengths, risks, and service needs.
- Develop, implement, and monitor individualized service plans in collaboration with clients and families.
- Provide short-term counseling and crisis intervention as needed, using evidence-based approaches.
- Coordinate referrals to housing, employment, healthcare, and legal services while ensuring timely follow-up.
- Advocate for clients within systems (schools, courts, healthcare) to improve access and outcomes.
- Maintain accurate, compliant case records and documentation in accordance with agency policies and regulatory standards.
- Collaborate with multidisciplinary teams to support holistic care and continuity of services.
- Engage in community outreach and program development to expand resources and support networks.
Qualification
- Masterβs degree in Social Work (MSW) from an accredited program; current state LCSW license preferred
- Minimum 2-4 years of professional social work experience in a community-based or clinical setting
- Strong assessment, counseling, and crisis intervention skills
- Experience with case management, care coordination, and resource navigation
- Excellent written and verbal communication, documentation, and ethical decision-making
- Ability to work collaboratively with diverse populations; culturally responsive practice
- Knowledge of relevant laws, regulations, and reporting requirements; proficiency with electronic health records
- Commitment to trauma-informed care, client empowerment, and social justice principles
