HopeBridge Community Services is seeking a compassionate and results-driven Licensed Social Worker to join our Family Services team in Chicago, IL. This full-time role focuses on delivering evidence-based social work services to at-risk families and individuals, coordinating resources, and advocating for client needs within a collaborative care model.
As a frontline member of our team, you will assess client needs, develop step-by-step service plans, connect clients with community resources, and monitor progress to achieve measurable outcomes. You will work closely with multidisciplinary professionals to ensure culturally competent, ethical, and person-centered care.

Responsibility
- Conduct comprehensive psychosocial assessments and develop individualized service plans.
- Coordinate and link clients to community resources, housing, employment, health care, and legal services.
- Provide crisis intervention and ensure safety planning for high-risk clients.
- Advise and support families through advocacy, counseling, and case management.
- Document interactions, maintain accurate records, and ensure compliance with agency policies and regulatory requirements.
- Collaborate with multidisciplinary teams to monitor progress and adjust treatment plans as needed.
- Engage in culturally responsive practices and maintain ethical standards in all interactions.
Qualification
- Bachelor’s or Master’s degree in Social Work (MSW preferred) from an accredited program.
- Current licensure or eligibility to practice as LMSW/LCSW in Illinois; valid social work license is preferred.
- Minimum 2 years of direct social work experience in family services, child welfare, or related fields.
- Strong assessment, counseling, and crisis intervention skills.
- Excellent written and verbal communication, with the ability to document clearly and accurately.
- Ability to work collaboratively with diverse populations and community partners.
- Proficiency in Microsoft Office, case management software, and community resource navigation.
- Commitment to ethical practice and a client-centered, trauma-informed approach.
