As a valued member of our multidisciplinary team, you will collaborate with physicians, case managers, and community partners to develop client-centered care plans, coordinate services, and document progress in a compliant electronic health record system.

Responsibility
- Conduct comprehensive biopsychosocial assessments and develop person-centered care plans.
- Provide counseling and crisis intervention as needed.
- Assist clients with accessing housing, financial, legal, and healthcare resources.
- Coordinate with multidisciplinary teams and service providers.
- Advocate for clients' rights and needs within the community and system.
- Maintain timely, compliant documentation in electronic health record systems.
- Develop and deliver community outreach and referral initiatives.
- Evaluate program outcomes and contribute to quality improvement.
Qualification
- Master's degree in Social Work (MSW) from an accredited program.
- Licensed or eligible for LSW in the state of Washington.
- Minimum 2 years of direct social work experience in healthcare, mental health, or community services.
- Strong assessment, counseling, and crisis intervention skills.
- Knowledge of community resources, housing, benefits programs, and entitlements.
- Excellent written and verbal communication; proficient documentation in EHR.
- Ability to work independently and collaboratively; culturally competent.
- Bilingual (Spanish/English) a plus.
