As a licensed social worker you will join a multidisciplinary team to assess needs, coordinate resources, and advocate for clients across systems including healthcare, housing, education, and child welfare. Responsibility
- Conduct comprehensive psychosocial assessments to inform person centered care planning and service delivery.
- Develop, implement, and monitor individualized care plans in collaboration with clients and families.
- Provide crisis intervention and coordinate emergency services as needed to ensure client safety.
- Coordinate housing assistance, financial supports, healthcare access, and community resources to remove barriers to stability.
- Advocate for clients with schools, healthcare providers, and social service agencies to ensure equitable access to services.
- Maintain accurate documentation in the electronic records system and ensure compliance with confidentiality and ethics standards.
- Facilitate group and individual counseling sessions using trauma informed and strengths based approaches.
- Collaborate with therapists, nurses, social workers, and community partners to deliver integrated care.
Qualification
- Master of Social Work MSW from an accredited program
- Licensed Social Worker LSW in Washington state or eligible to obtain licensure within 3 months
- Minimum of 2 years of clinical social work or case management experience
- Strong assessment, crisis intervention, and trauma informed care skills
- Excellent written and verbal communication and cultural humility
- Experience with EMR systems and documentation standards
- Knowledge of community resources, housing programs, and social services
- Ability to work collaboratively in a fast paced, client centered environment
