Responsibility
- Verify and dispense prescriptions with precision, ensuring safety and regulatory compliance.
- Counsel patients on medications, potential side effects, interactions, and adherence strategies.
- Collaborate with physicians and the care team on medication therapy management plans.
- Monitor medication safety programs, report adverse events, and implement quality improvements.
- Oversee compounding and preparation of select sterile and non-sterile products (where applicable).
- Maintain accurate documentation in the Pharmacy Information System and ensure optimal inventory management.
- Ensure compliance with state and federal regulations, licensing requirements, and accreditation standards.
Qualification
- Active Pharmacist license in California (or eligibility) and PharmD degree from an accredited program.
- Minimum 2 years of clinical or retail pharmacy experience in a patient-facing role.
- Strong knowledge of pharmacology, drug interactions, and evidence-based guidelines.
- Excellent communication and counseling skills with a patient-centered approach.
- Proficiency with Pharmacy Management Systems and e-prescribing; ability to utilize data for outcomes improvement.
- Detail-oriented, with strong problem-solving and time-management abilities in a fast-paced environment.
