VitalRx Pharmacy is seeking a licensed Pharmacist to join our progressive team in San Francisco, California. This role emphasizes patient-centered care, medication safety, and clinical collaboration with physicians and technicians.
In this full-time position, you will ensure accurate dispensing, provide medication counseling, review regimens for safety and efficacy, and support peri- and post-discharge transitions of care. You’ll work with state-of-the-art pharmacy software and participate in quality improvement initiatives to enhance patient outcomes.

Responsibility
- Verify and dispense prescriptions accurately while ensuring patient safety and regulatory compliance.
- Provide clinical counseling on medications, dosing, adverse effects, interactions, and adherence strategies.
- Review patient profiles for drug interactions, allergies, and contraindications; communicate with prescribers as needed.
- Participate in medication therapy management and chronic disease management programs.
- Support immunization services, vaccine administration, and public health initiatives where applicable.
- Maintain accurate documentation and ensure HIPAA-compliant handling of patient information.
- Collaborate with pharmacists, technicians, and care teams to optimize workflow and minimize wait times.
- Engage in quality assurance activities, audits, and process improvements.
Qualification
- Doctor of Pharmacy (PharmD) or equivalents licensed to practice in California.
- California Pharmacist License in good standing or eligible for licensure.
- Minimum 3 years of clinical or community pharmacy experience preferred.
- Strong knowledge of pharmacology, pharmacokinetics, and drug interactions.
- Excellent communication, patient counseling, and empathetic service skills.
- Proficiency with pharmacy information systems, e-prescribing, and data entry; basic computer literacy.
- Commitment to patient safety, regulatory compliance, and continuous learning.
- Ability to work flexible shifts, including evenings and weekends as needed.
