Responsibility
- Verify, dispense, and reconcile medications according to physician orders and applicable laws and regulations.
- Provide patient-focused counseling to ensure safe and effective use of prescribed therapies.
- Collaborate with clinicians to optimize therapeutic outcomes and identify potential drug interactions or contraindications.
- Review and manage medication profiles for accuracy, safety, and cost-effectiveness.
- Support compounding, sterile preparation, and formulary management as appropriate to the setting.
- Maintain precise documentation and ensure compliance with regulatory standards and quality assurance processes.
- Educate peers and staff on new therapies, safety initiatives, and clinical guidelines.
Qualification
- Active pharmacist license in Washington state or eligible for licensure.
- Doctor of Pharmacy (PharmD) or equivalent clinical degree.
- Minimum 2 years of hospital, community, or specialty pharmacy experience preferred.
- Strong knowledge of pharmacology, medication safety, and drug utilization review.
- Excellent communication skills and a patient-centered approach.
- Demonstrated proficiency with Pharmacy Information Systems and electronic health records.
- Ability to work collaboratively in a multidisciplinary team and adapt to a fast-paced environment.
- Strong problem-solving, attention to detail, and organizational skills.
