Responsibility
- Manage and coordinate comprehensive care plans for clients undergoing early psychosis intervention.
- Conduct thorough assessments of patients’ psychosocial needs, risk factors, and treatment progress.
- Provide counseling, psychoeducation, and support for patients and their families throughout the recovery process.
- Work collaboratively with psychiatrists, psychologists, nurses, and allied health professionals to ensure holistic care delivery.
- Monitor and document patients’ progress, adjust interventions as needed, and ensure adherence to therapeutic plans.
- Facilitate referrals to relevant community resources, social services, or support groups as appropriate.
- Participate in case conferences, clinical supervision, and continuous professional development activities.
- Advocate for patient welfare and contribute to mental health awareness initiatives within the community.
Qualification
- Bachelor’s Degree or higher in Social Work, Psychology, Nursing, Counselling, or a related healthcare discipline.
- At least 3-5 years of relevant case management experience, preferably in a psychiatric or mental health setting.
- Strong clinical assessment, problem-solving, and crisis intervention skills.
- Excellent communication, interpersonal, and teamwork abilities.
- Knowledge of early psychosis, psychiatric rehabilitation, and community mental health resources.
- Certification or registration with relevant professional bodies is an advantage.
- Demonstrated ability to work sensitively with individuals and families from diverse backgrounds.
- Proficiency in documentation and use of healthcare information systems.
