BrightPath Community Services is seeking a licensed social worker to join our Chicago team. This full time role focuses on delivering compassionate case management, crisis intervention, and outcome driven support to diverse clients. You will collaborate with housing, healthcare, education, and legal partners to connect clients with essential resources. We value equity, trauma informed care, and ethical practice.
Responsibility
- Provide comprehensive case management for diverse client populations, including intake, assessment, planning, and follow-up.
- Conduct risk assessments and crisis intervention to ensure client safety.
- Develop individualized service plans and coordinate with community resources and service providers.
- Advocate for clients to access housing, healthcare, education, and financial assistance.
- Maintain accurate documentation in electronic health records in compliance with HIPAA and organizational policies.
- Collaborate with multidisciplinary teams to support client centered care and outcomes.
- Deliver culturally competent services and participate in ongoing training and supervision.
Qualification
- Master of Social Work MSW from an accredited program; active LSW or LISW preferred.
- Minimum of 2 years of social work experience in health, housing, or family services.
- Strong knowledge of community resources, housing systems, healthcare access, and social supports.
- Excellent communication, assessment, and crisis intervention skills.
- Ability to work independently and within a multidisciplinary team; proficient in documentation and ethics including HIPAA.
- Demonstrated commitment to equity, cultural humility, and trauma-informed care; bilingual English/Spanish is a plus.
