Responsibility
- Conduct comprehensive biopsychosocial assessments to identify client needs and goals.
- Develop and monitor individualized service plans, coordinating with multidisciplinary teams.
- Connect clients with housing, employment, healthcare, and mental health resources.
- Provide supervision, coaching, and professional development for a team of case managers.
- Ensure compliant documentation, data integrity, and adherence to privacy regulations.
- Advocate for clients and families within the community and with partner agencies.
- Utilize data and feedback to drive program improvements and outcomes.
Qualification
- Bachelor's degree in social work, psychology, sociology, or related field; MSW preferred.
- Minimum 3 years of direct service experience in social services, with supervisory experience preferred.
- Strong knowledge of community resources and intake processes.
- Excellent communication, cultural humility, and conflict resolution skills.
- Ability to conduct risk assessments, crisis intervention, and safety planning.
- Proficiency with case management software, data reporting, and HIPAA compliance.
- Fluency in English; Spanish or other languages a plus.
