Harborview Community Services is seeking a compassionate and resultsâdriven Senior Social Work Case Manager to join our Boston team. In this role, you will partner with clients and families to assess needs, build durable service plans, and connect them to essential resources. Responsibility
- Conduct comprehensive biopsychosocial assessments and risk evaluations to inform individualized service plans.
- Coordinate case management across a multidisciplinary team to ensure seamless service delivery.
- Advocate for clients to access housing, healthcare, benefits, and community resources aligned with their goals.
- Provide crisis intervention, safety planning, and ongoing risk management as needed.
- Document case notes, goals, outcomes, and regulatory compliance in the electronic health records system.
- Monitor client progress, adjust plans based on changes in needs, and report outcomes to stakeholders.
- Engage families, caregivers, and community partners to build supportive networks and strengthen service coordination.
Qualification
- Masterâs degree in Social Work (MSW) from a CSWEâaccredited program.
- LCSW or LMSW licensure in Massachusetts, or eligibility to obtain licensure.
- Minimum of 3 years of relevant case management or clinical social work experience.
- Strong knowledge of traumaâinformed care, culturally responsive practices, and strengthsâbased approaches.
- Excellent assessment, documentation, communication, and advocacy skills.
- Proficiency with electronic health records (EHR) and HIPAA compliance.
- Ability to work effectively in a fastâpaced environment, manage competing priorities, and travel within the service area as needed.
