Responsibility

  • Manage a diverse caseload of clients across ages and backgrounds, conducting comprehensive assessments and developing individualized service plans.
  • Provide ongoing case management and direct advocacy to connect clients with housing, healthcare, education, legal, and social supports.
  • Conduct psychosocial assessments and risk screens, implement safety planning, and coordinate crisis intervention when needed.
  • Collaborate with interdisciplinary teams, including clinicians, educators, and community partners, to ensure integrated care and timely referrals.
  • Document progress in the electronic health record, maintain compliance with program standards, and monitor outcomes against measurable targets.
  • Support families and individuals with goal setting, empowerment, and skills development to promote independence and stability.
  • Advocate for clients in systems navigation, policy barriers, and access to culturally responsive services.

Qualification

  • Master of Social Work MSW from an accredited program; eligible for licensure in Washington state, LCSW or LMSW preferred.
  • Minimum 2 years of clinical or case management experience in health care, child welfare, or community services.
  • Strong knowledge of trauma informed care, behavioral health, and community resources.
  • Excellent communication, cultural humility, and client centered interviewing skills.
  • Proficiency with electronic health records and case management software; ability to document accurately and timely.
  • Ability to work independently and as part of a multidisciplinary team; strong organizational and time management skills.
  • Valid driver's license and access to reliable transportation may be required for field visits.
