As a Social Services Case Manager, you will coordinate access to housing, food assistance, mental health services, and supportive programs while advocating for client rights and autonomy.
Responsibility
- Conduct comprehensive intake assessments to determine client needs and eligibility for services.
- Develop personalized service plans in collaboration with clients and multidisciplinary teams.
- Coordinate referrals to housing, healthcare, cash assistance, and community-based programs.
- Advocate for clients with partner agencies, ensuring timely access to resources.
- Monitor progress, document outcomes, and maintain case notes in the EMR.
- Provide crisis intervention and de-escalation when needed and connect clients to emergency resources.
- Maintain adherence to privacy, confidentiality, and ethical guidelines.
- Participate in team meetings, trainings, and program evaluation activities.
Qualification
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field (MSW preferred).
- Minimum of 2 years of experience in case management or social services.
- Strong communication, active listening, and cultural competency skills.
- Knowledge of community resources and eligibility requirements for public programs.
- Proficiency with electronic medical records (EMR) and case management software.
- Ability to work independently and collaboratively in a fast-paced environment.
- Demonstrated crisis intervention and problem-solving abilities.
- Bilingual in English and Spanish is a plus.
