Job description

As a Case Manager you will partner with clients to assess needs, connect them with essential services, and advocate for fair access to resources including housing, healthcare, employment, and education. Responsibility

  • Conduct comprehensive psychosocial assessments and develop individualized service plans in collaboration with clients.
  • Coordinate and connect clients with housing, healthcare, employment, substance use treatment, and mental health services.
  • Provide ongoing case management, crisis intervention, advocacy, and follow-up to ensure service delivery.
  • Maintain accurate, timely documentation in electronic records and case management systems.
  • Monitor client progress, adjust plans as needed, and report outcomes to supervisory staff.
  • Build and maintain positive relationships with community partners and service providers.
  • Facilitate referrals, benefits applications (SSI/SSDI, SNAP, TANF), and enrollment in support programs.
  • Ensure compliance with agency policies, confidentiality standards, and relevant regulations.

Qualification

  • Bachelor's degree in Social Work, Psychology, Sociology, or a related field; MSW preferred.
  • 3+ years of case management or social services experience, preferably in a community-based setting.
  • Knowledge of local Boston-based resources and housing/benefits programs.
  • Strong communication, interpersonal, and crisis intervention skills; trauma-informed care approach.
  • Proficiency with EMR and electronic case management systems; excellent documentation skills.
  • Ability to work independently, prioritize, and manage a caseload with empathy and professionalism; occasional evenings or weekends may be required.
  • Bilingual in English and Spanish preferred; other languages considered.
  • Valid driver’s license or reliable transportation for outreach as needed.
