Join CommunityBridge Alliance as a Social Services Case Manager in Portland, Oregon. This full-time role supports individuals and families navigating housing, benefits, healthcare, and community resources. You will conduct assessments, develop person-centered care plans, coordinate services, and advocate for clients to improve outcomes and stability.

Responsibility
- Conduct comprehensive client assessments to determine needs, strengths, and risk factors, and document findings in our electronic case management system.
- Develop and implement individualized service plans in collaboration with clients, families, and multidisciplinary teams.
- Coordinate access to housing, financial assistance, healthcare, childcare, and other community resources; assist clients with benefit applications and renewals.
- Provide crisis intervention, safety planning, and de-escalation as needed, following established protocols.
- Advocate for clients in meetings with service providers, housing authorities, schools, and courts to secure needed supports.
- Monitor progress, adjust plans based on outcomes, and maintain timely, accurate documentation and reporting.
- Collaborate with colleagues to ensure trauma-informed, culturally responsive service delivery and data-driven program improvements.
Qualification
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field; Master’s degree preferred or equivalent professional experience.
- 2+ years of experience in social services, case management, or related field; knowledge of local resources in Portland is a plus.
- Strong interpersonal and communication skills; ability to build trust with diverse clients and families.
- Knowledge of benefits navigation, housing resources, healthcare access, and community supports; proficient with public benefits systems.
- Excellent organizational skills with attention to detail and ability to manage multiple cases simultaneously.
- Experience with electronic case management systems and demonstrated commitment to confidentiality and privacy (HIPAA-compliant practices).
- Ability to work independently and as part of a multidisciplinary team; flexible schedule including some evenings or weekends as needed.
- Preferred bilingual proficiency in Spanish, Vietnamese, or other languages relevant to the community.
