Join CivicCare Community Services as a Social Services Case Manager. This mission-driven role supports individuals and families in Baltimore, helping them access housing, healthcare, food resources, and educational opportunities. You will conduct client assessments, develop strength-based service plans, and coordinate with partner agencies to connect clients with essential supports. Responsibility
- Conduct comprehensive client assessments and develop individualized service plans based on identified needs and strengths.
- Coordinate referrals to housing, employment, healthcare, mental health, legal aid, and other community resources.
- Advocate on behalf of clients to overcome barriers and ensure access to benefits and services.
- Maintain accurate, timely case notes in the client management system and ensure documentation meets regulatory requirements.
- Monitor progress and outcomes, adjust plans as needed, and report on case status to supervisors.
- Collaborate with social workers, nurses, educators, and community partners to deliver integrated services.
- Provide crisis intervention and de-escalation support when needed, connecting clients to appropriate resources.
- Promote client-centered approaches that respect dignity, privacy, and cultural differences.
Qualification
- Bachelors degree in Social Work, Psychology, Sociology, or a related field (MSW preferred).
- Minimum 2 years of case management or direct service experience in a social services setting.
- Knowledge of community resources, housing programs, benefits, and assistance options available in Baltimore.
- Strong interpersonal and communication skills, with demonstrated ability to build rapport and collaborate with diverse clients.
- Excellent organizational skills, attention to detail, and proficient documentation in electronic case management systems.
- Ability to assess risk, provide crisis intervention, and maintain composure in challenging situations.
- Bilingual in English and Spanish or other languages preferred.
- Commitment to equity, client rights, and ethical guidelines in social services practice.
