Job description

Harborview Community Services is seeking a compassionate and results-driven Social Services Case Manager to join our Chicago team. Responsibility

  • Conduct comprehensive client assessments and develop person-centered service plans.

  • Advocate on behalf of clients with agencies, schools, landlords, and employers to ensure access to rights and services.

Qualification

  • Bachelor's degree in social work, psychology or related field; relevant experience may substitute for a degree.
  • Minimum 2 years of case management or social services experience, preferably with diverse populations.
  • Strong knowledge of local social services resources in Chicago and surrounding counties.
  • Excellent interpersonal, communication, and culturally competent skills.
  • Crisis intervention and de-escalation skills; ability to handle confidential information with discretion.
  • Proficiency in case management software and MS Office; data-driven with strong documentation practices.
  • Ability to work flexible hours including occasional evenings or weekends; valid driver's license may be required.
  • Spanish bilingual skills are desirable but not required.
