Harborview Community Services is seeking a compassionate and results-driven Social Services Case Manager to join our Chicago team. You will partner with clients and families to assess needs, connect them with essential resources, and advocate for access to housing, financial assistance, healthcare, and educational supports. Responsibility
- Conduct comprehensive client assessments and develop person-centered service plans.
- Coordinate and connect clients with community resources including housing, benefits, healthcare, and education.
- Provide trauma-informed counseling and crisis support, referring to specialized services as needed.
- Advocate on behalf of clients with agencies, schools, landlords, and employers to ensure access to rights and services.
- Monitor progress, document case notes, and measure outcomes to adjust plans.
- Maintain accurate records in the case management system and ensure confidentiality and data integrity.
- Collaborate with an interdisciplinary team and participate in case conferences to coordinate care.
Qualification
- Bachelor's degree in social work, psychology or related field; relevant experience may substitute for a degree.
- Minimum 2 years of case management or social services experience, preferably with diverse populations.
- Strong knowledge of local social services resources in Chicago and surrounding counties.
- Excellent interpersonal, communication, and culturally competent skills.
- Crisis intervention and de-escalation skills; ability to handle confidential information with discretion.
- Proficiency in case management software and MS Office; data-driven with strong documentation practices.
- Ability to work flexible hours including occasional evenings or weekends; valid driver's license may be required.
- Spanish bilingual skills are desirable but not required.
