As a Case Manager, you will conduct comprehensive assessments, develop individualized service plans, and coordinate with partner agencies. Responsibility
- Conduct comprehensive client assessments to determine eligibility and needs for services.
- Develop and implement individualized service plans and monitor progress against goals.
- Coordinate referrals to housing, financial assistance, healthcare, and community resources.
- Advocate on behalf of clients with agencies, employers, and landlords to improve access to benefits and services.
- Provide crisis intervention, de-escalation, and supportive counseling when necessary.
- Document case notes, outcomes, and regulatory compliance in our case management system.
- Collaborate with a multidisciplinary team to ensure holistic, client-centered care.
- Engage with clients through home visits and community outreach as appropriate.
Qualification
- Bachelors degree in Social Work, Sociology, Psychology, or related field; MSW preferred.
- Minimum 2 years of case management or social services experience.
- Strong knowledge of community resources, benefits programs, and eligibility criteria.
- Excellent communication, interviewing, and active listening skills.
- Ability to manage multiple cases, organize documentation, and meet deadlines.
- Proficiency with electronic case management systems and data entry.
- Compassion, cultural humility, resilience, and crisis intervention abilities.
- Valid driver’s license and willingness to conduct occasional field visits (if required).
