Join BrightPath Community Services as a dedicated Social Services Case Manager in Portland, Oregon. You will support families and individuals by connecting them with essential resources, developing person centered plans, and advocating for client needs within a collaborative, high impact team.
This role emphasizes compassionate client engagement, eligibility navigation, and cross agency collaboration to deliver holistic support that improves stability and outcomes for the people we serve.
Responsibility
- Conduct comprehensive assessments to determine client needs, eligibility, and risk factors.
- Develop individualized service plans and monitor progress against goals.
- Coordinate referrals to housing, healthcare, counseling, financial assistance, employment services, and child welfare supports.
- Advocate for clients with public agencies, schools, housing providers, and community organizations.
- Document case notes accurately in the electronic case management system while maintaining strict confidentiality.
- Collaborate with multidisciplinary teams to ensure integrated services and follow-up.
- Provide crisis response and safety planning; escalate situations per policy when necessary.
Qualification
- Bachelor degree in social work or related field; MSW preferred
- Minimum 2 years of case management or social services experience
- Strong understanding of community resources and eligibility criteria
- Excellent communication, organizational, and time management skills
- Ability to work with diverse populations with empathy and respect
- Proficiency with electronic case management systems and data entry
- Valid driver's license and willingness to travel within the service area
- Commitment to ethical practice and confidentiality
