BrightPath Community Services is seeking a compassionate and detail-oriented Social Services Case Manager to support families and individuals in the Chicago area. Responsibility
- Conduct comprehensive needs assessments and develop individualized service plans.
- Coordinate with healthcare, housing, education, and employment resources to support clients.
- Monitor progress, document service delivery, and adjust plans as needed.
- Provide crisis intervention and ongoing case management for at-risk clients.
- Advocate for client access to benefits and community supports.
- Maintain compliance with program policies, privacy regulations, and reporting requirements.
- Collaborate with multidisciplinary teams to ensure holistic client care.
Qualification
- Bachelor's degree in Social Work, Psychology, or related field; MSW preferred.
- Active state social work license or equivalent credential preferred.
- 2+ years of case management or social services experience.
- Strong communication, empathy, and client advocacy skills.
- Knowledge of community resources, housing, and benefits programs.
- Trauma-informed approach and crisis intervention experience.
- Proficiency with case management software and documentation standards.
