HopeBridge Community Services is seeking a dedicated Social Services Case Manager to support individuals and families in navigating complex service systems. This full time role combines client-centered assessments, resource referral, and collaborative care planning to promote safety, stability, and independence. You will work with a multidisciplinary team, community partners, and clients to deliver high quality social services in a fast paced urban environment.

Responsibility
- Conduct comprehensive client assessments to determine needs, risk factors, and eligibility for services.
- Develop individualized service plans in collaboration with clients and caregivers, monitoring progress and outcomes.
- Coordinate access to housing, healthcare, food assistance, education, and income supports; advocate for clients with partner agencies.
- Provide crisis intervention and de-escalation as needed, ensuring safety and referrals to appropriate resources.
- Maintain accurate, confidential case notes and documentation in compliance with agency policies and legal requirements.
- Engage community resources and develop referral networks to expand service access.
- Participate in team meetings, supervision, and professional development; contribute to program improvement initiatives.
Qualification
- Master's or Bachelor's degree in Social Work, Psychology, Sociology, or related field; MSW preferred.
- Minimum 2 years of social services or case management experience in nonprofit or public sector.
- Strong interviewing, assessment, and solution-focused counseling skills.
- Knowledge of local social service systems, eligibility criteria, and community resources.
- Excellent communication, documentation, and crisis intervention abilities.
- Demonstrated ability to manage complex caseloads with strong time management and organization.
- Commitment to culturally competent, client-centered, trauma-informed practice; valid driver's license may be preferred.
