Responsibility
- Conduct comprehensive client intake and needs assessments to determine eligibility and service priorities.
- Develop individualized service plans with measurable goals and track progress over time.
- Coordinate and monitor services across housing, benefits, healthcare, and community resources.
- Advocate on behalf of clients to secure timely access to supports and remove barriers.
- Maintain accurate case files, prepare progress notes, and ensure compliance with privacy and program guidelines.
- Provide crisis assessment and basic de-escalation strategies, with coordinated referrals as needed.
- Engage in community outreach to raise awareness of available resources and build partnerships.
- Collaborate with multidisciplinary teams, supervisors, and interns to ensure holistic client care and service continuity.
Qualification
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field; LSW preferred or willing to pursue licensure.
- Minimum of 2 years of progressive experience in social services or case management.
- Strong knowledge of community resources, housing programs, benefits systems, and healthcare access.
- Exceptional communication and documentation skills with a client-centered approach.
- Proven crisis intervention and de-escalation abilities; comfortable handling sensitive situations.
- Ability to manage a dynamic caseload with organization, time management, and attention to detail.
- Proficiency with case management software, data entry, and Microsoft Office; willingness to learn new tools.
- Valid driver’s license and reliable transportation preferred for client home visits or community outreach.
