As a Case Manager you will assess client needs, develop service plans, connect clients to community resources, and advocate on their behalf with housing authorities, public benefits, and healthcare providers. You will collaborate with a multidisciplinary team to ensure services are client centered, culturally competent, and trauma informed.

Responsibility

  • Coordinate comprehensive case management for a diverse client population including intake, assessment, and ongoing planning.
  • Develop individualized service plans with measurable goals and timelines.

Qualification

  • Bachelor degree in social work, psychology, sociology, or a related field; MSW preferred.
  • Minimum 2 years of relevant experience in social services or case management.
  • Strong knowledge of community resources, benefits programs, and housing systems.
  • Excellent communication, empathy, and crisis intervention skills.
  • Ability to manage competing priorities and work independently as well as in a team.
  • Proficiency with case management software and data entry.
  • Fluency in Spanish is a plus; willingness to work with diverse populations.
  • Commitment to trauma-informed, client-centered practice and ethical standards.
