Hopewell Community Services is seeking a compassionate and proactive Social Services Case Manager to join our Chicago team. This full-time role supports individuals and families facing housing insecurity, poverty, and other challenges by coordinating services, assessing needs, and advocating for client outcomes within a trauma-informed, client-centered framework.
In this role you will:
- Conduct comprehensive client assessments and develop individualized service plans.
- Coordinate housing, employment, education, and health services with partner agencies.
- Provide crisis intervention and safety planning as needed.
- Document interactions and outcomes in our case management system with accuracy and timeliness.
- Advocate for clients to access community resources and remove barriers to success.
- Collaborate with multidisciplinary teams to monitor progress and adjust plans.
- Maintain ethical standards, confidentiality, and cultural sensitivity in all activities.
- Support program evaluation by contributing to data collection and reporting.
Qualifications and experience that help you succeed include a bachelor's degree in social work, psychology, or a related field, plus 2+ years of case management or related experience. A passion for social justice, strong communication skills, and the ability to balance compassion with accountability are essential.

Qualification
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field; Master’s degree preferred
- Minimum 2 years of case management or direct client service experience in a social services setting
- Strong knowledge of community resources and eligibility requirements
- Excellent communication, active listening, and crisis intervention skills
- Proficiency with electronic case management systems and documentation best practices
- Ability to work independently and collaboratively within a multidisciplinary team
- Commitment to anti-bias practices, cultural humility, and trauma-informed care
- Eligibility to work in the United States; valid driver’s license may be required for field visits
