In this role, you will be responsible for assessing client needs, developing care plans, coordinating with community partners, and monitoring progress to promote safety, well-being, and independence.
Responsibility
- Conduct comprehensive needs assessments and develop individualized service plans for clients.
- Coordinate housing, food, healthcare, mental health, and employment resources with partner agencies.
- Provide ongoing case management, crisis intervention, and advocacy to clients and families.
- Document interactions, service plans, progress notes, and outcomes in the agency's case management system.
- Monitor progress, evaluate outcomes, and adjust plans to ensure client goals are met.
- Collaborate with multidisciplinary teams and participate in supervision and quality improvement initiatives.
Qualification
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field; LSW/LSW-eligible preferred.
- Minimum 2 years of direct client services in social services or non-profit settings.
- Knowledge of community resources, public benefits, housing and healthcare systems.
- Strong communication, active listening, and interpersonal skills; ability to advocate for clients.
- Proficiency with case management software and data entry; excellent documentation practices.
- Ability to work independently and as part of a team; crisis intervention skills preferred.
- Commitment to anti-racism, equity, and client-centered practices.
