Responsibility
- Conduct comprehensive client assessments to determine needs, risks, and strengths.
- Develop, implement, and monitor individualized service plans with measurable goals and timelines.
- Coordinate access to housing, healthcare, financial assistance, and education resources.
- Advocate for clients with social service agencies, schools, and healthcare providers.
- Maintain accurate case notes, documentation, and regulatory compliance.
- Provide crisis intervention and de-escalation support when needed.
- Collaborate with social workers, psychologists, and community partners to ensure holistic support.
- Track progress, advocate for program improvements, and support client independence.
Qualification
- Bachelor's degree in Social Work, Human Services, or a related field; MSW or LCSW preferred.
- Minimum 2 years of case management or direct service experience in social services.
- Strong knowledge of local community resources in Portland and Multnomah County.
- Excellent assessment, planning, documentation, and organizational skills.
- Exceptional communication, empathy, and cultural competence abilities.
- Valid driver's license and reliable transportation for field visits (if required).
- Ability to work independently and collaboratively within a multidisciplinary team.
- Fluency in Spanish or other languages is a plus.
