NorthBridge Community Services is seeking a dedicated Social Services Case Manager to assess, plan, and coordinate services for clients facing housing instability, unemployment, or family crisis. Responsibility
- Conduct comprehensive client intake assessments and risk screening to determine eligibility and needs.
- Develop individualized service plans in collaboration with clients, families, and community partners.
- Coordinate multidisciplinary supports (housing, employment, health, legal aid) to facilitate durable outcomes.
- Provide crisis intervention and de-escalation support as needed.
- Advocate on behalf of clients with agencies and service providers to secure resources.
- Maintain precise documentation in case management systems and ensure compliance with privacy regulations.
- Monitor progress, adjust plans, and conduct regular case reviews with supervisors.
- Deliver trauma-informed, culturally competent services to diverse client populations.
Qualification
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field; Master’s preferred.
- Active LSW/LCSW or progress toward licensure preferred; relevant certifications welcomed.
- Minimum 2 years of experience in social services, case management, or related roles.
- Strong interpersonal and communication skills with ability to build trust with clients and partners.
- Knowledge of community resources, housing programs, financial assistance, and childcare services.
- Proficiency with case management software, data entry, and reporting.
- Ability to handle sensitive information with confidentiality; crisis intervention skills a plus.
- Bilingual abilities (English/Spanish or English/other) highly valued.
