HopeWell Community Services is seeking a proactive Social Services Program Coordinator to lead and coordinate community-based programs that support families and individuals in need. This role blends case management, outreach, and collaboration with partners to deliver high-impact services that promote housing stability, access to benefits, mental health support, and child and family well-being.
In this role, you will work closely with clients, families, and partner organizations to assess needs, develop action plans, and monitor progress. You will also ensure compliance with funding requirements and maintain accurate program data to demonstrate impact.

Responsibility
- Coordinate and implement community-based programs addressing housing stability, food security, and access to mental health services.
- Develop partnerships with local schools, clinics, nonprofits, and government agencies to expand service reach.
- Conduct intake assessments, design individualized care plans, and monitor client progress and outcomes.
- Assist clients with benefits enrollment, referrals to internal and community resources, and navigation of complex systems.
- Ensure program compliance, accurate data reporting, and timely submission of grant deliverables.
- Supervise and mentor a small team of case managers, interns, and volunteers; provide ongoing coaching and performance feedback.
- Plan and lead community workshops, outreach events, and culturally responsive services for diverse populations.
Qualification
- Bachelor's degree in Social Work, Sociology, Psychology, or a related field; MSW preferred.
- Minimum 3 years of experience in social services, case management, or community outreach.
- Strong knowledge of local social services resources, housing programs, and public benefits.
- Excellent communication, assessment, and crisis-intervention skills with a trauma-informed approach.
- Ability to work with diverse populations and demonstrate cultural humility and inclusivity.
- Proficiency with case management software, data collection, and reporting; strong organizational skills.
- Valid driver's license and reliable transportation; ability to travel within the service area as needed.
