We are seeking a compassionate and results-driven Social Services Professional to join Harborview Community Services in Chicago, IL. You will oversee client programs, coordinate case management, and advocate for community needs to empower individuals and families.

Responsibility
- Lead and coordinate case management for diverse client populations, ensuring individualized service plans.
- Develop and implement community-based social services programs in alignment with agency goals.
- Collaborate with cross-functional teams to assess client needs and remove barriers to service access.
- Monitor program outcomes, collect data, and prepare progress reports for stakeholders.
- Provide crisis intervention, de-escalation, and problem-solving support.
- Ensure compliance with funding requirements and agency policies; manage documentation and confidentiality.
- Advocate for clients and connect them with internal and external resources to support stability and growth.
Qualification
- Bachelor's degree in social work, psychology, sociology, or related field; Master’s degree preferred.
- Licensure or certification such as LCSW/LMSW preferred; willingness to obtain within 12 months.
- Minimum 2-4 years of experience in social services, case management, or related field.
- Strong communication, empathy, and crisis intervention skills.
- Proven ability to manage multiple cases with attention to detail and compliance.
- Proficiency with case management software and data entry; solid reporting abilities.
- Knowledge of local community resources and referral networks; culturally competent approach.
