You will provide compassionate, client-centered support to individuals and families, conduct comprehensive assessments, and coordinate with community resources to promote sustainable outcomes. Responsibility
- Conduct biopsychosocial assessments to identify needs, strengths, and risks for clients and families.
- Develop and implement person-centered care plans in collaboration with clients and care teams.
- Provide counseling, crisis intervention, and ongoing case management to promote stability and independent functioning.
- Coordinate access to housing, healthcare, education, employment, and social services; advocate for client rights and resources.
- Collaborate with interdisciplinary teams, community partners, and supervisors to ensure coordinated care.
- Document case notes, progress, and outcomes in electronic records and ensure compliance with regulatory standards.
- Monitor client safety, assess risk, and implement appropriate interventions or referrals as needed.
Qualification
- Master's degree in Social Work (MSW) from an accredited program; LCSW or LMSW preferred.
- Minimum of 2 years of direct client services in social work, healthcare, or nonprofit settings.
- Strong assessment, counseling, and crisis intervention skills; experience with trauma-informed care.
- Knowledge of community resources and service systems; ability to navigate complex systems for client benefit.
- Excellent written and verbal communication skills; proficient documentation and reporting abilities.
- Ability to work effectively with diverse populations; commitment to ethics, confidentiality, and client-centered practice.
- Proficiency with electronic health records (EHR) and basic computer skills; strong organizational skills.
