BrightPath Community Services is actively seeking a compassionate and skilled Social Worker to join our Chicago team. This role focuses on case management, client advocacy, and connecting families to essential services.
As a Social Worker, you will conduct intake assessments, develop individualized service plans, and collaborate with community partners to support clients in overcoming barriers to housing, healthcare, education, and financial stability.

Responsibility
- Conduct psychosocial assessments and develop individualized service plans.
- Provide counseling, crisis intervention, and safety planning as needed.
- Coordinate access to housing, financial assistance, healthcare, and education resources.
- Collaborate with schools, medical providers, and community agencies to ensure seamless care.
- Advocate for client rights and navigate social service systems to maximize outcomes.
- Monitor progress, document case notes, and update service plans regularly.
- Participate in multidisciplinary teams and contribute to program development.
Qualification
- Masterβs degree in Social Work (MSW) or related field; LCSW/LICSW preferred.
- Licensed or eligible for licensure in Illinois.
- 2+ years of direct client service in social services or case management.
- Strong knowledge of community resources and eligibility for public assistance.
- Excellent communication, assessment, and advocacy skills.
- Crisis intervention experience and culturally competent practice.
- Proficiency with case management software and documentation standards.
- Commitment to equity, inclusion, and client-centered practice.
