Responsibility
- Conduct comprehensive psychosocial assessments and develop individualized service plans aligned with client goals.
- Provide proactive case management, coordinate services, and connect clients to housing, healthcare, benefits, and community resources.
- Offer crisis intervention and de-escalation support to ensure client safety and stabilize situations.
- Advocate for clients’ rights and access to needed services, including referrals to housing, education, employment, and legal assistance.
- Document case notes, progress, and outcomes in the electronic health/records system with accuracy and timeliness.
- Collaborate with physicians, nurses, social workers, and community partners to deliver holistic care.
- Monitor progress, adjust service plans as needed, and ensure compliance with program guidelines and regulations.
- Provide culturally competent services and maintain ethical boundaries while building trust with diverse populations.
Qualification
- MSW from an accredited program; LCSW or LMSW in NY with licensure or eligibility required.
- Minimum 2 years of post-graduate experience in clinical or community-based social work.
- Strong assessment and case management skills, including risk assessment and crisis intervention.
- Excellent written and verbal communication; proficient documentation in EMR systems.
- Ability to work effectively in a multidisciplinary team and manage a sensitive caseload.
- Demonstrated cultural humility and experience serving diverse populations.
- Knowledge of local resources, housing programs, benefits, and advocacy strategies.
- Commitment to ethical practice and confidentiality, with strong problem-solving skills.
