Responsibility
- Conduct comprehensive psychosocial assessments and risk screening for clients and families.
- Develop, implement, and monitor individualized service plans in collaboration with clients and team members.
- Provide crisis intervention, case management, and advocacy to access housing, benefits, and community resources.
- Coordinate referrals to mental health services, healthcare, education, and legal supports as needed.
- Document case notes accurately in electronic health records and ensure data privacy compliance.
- Partner with community organizations to build durable supports and improve client outcomes.
Qualification
- Master's degree in Social Work (MSW) from an accredited program.
- LCSW or LICSW license preferred, or eligibility to obtain within 6-12 months.
- Minimum 2 years of relevant experience in case management, counseling, or clinical social work.
- Strong psychosocial assessment, crisis intervention, and treatment planning skills.
- Solid knowledge of community resources and social service systems in the Seattle area.
- Excellent communication, cultural humility, and collaboration with multidisciplinary teams.
- Proficiency with electronic case notes, privacy practices, and outcome tracking.
